Extension Requests
Finding it difficult to submit an assessment item on time due to personal issues. Complete an Application for an Extension of Time for an Assessment Submit this form to the Lecturer in Charge of the unit.
You will need to attach documentation to verify your circumstances e.g.
- Medical certificate,
- If you are already seeing an ACU counsellor, documentation from an ACU counsellor.
- Documentation from another health professional
- Statutory Declaration completed by yourself, explain your own circumstances on the appropriate Statutory Declaration form, have it appropriately witnessed and attach to your extension request. Remember it is important for you to include a description of your circumstances, their impact and duration.
Important: You do not need to speak to a counsellor unless you would like counselling support.
